hermes - 💡(How to fix) Fix Adopt GitHub Issues as an agent task queue

Objective

Adopt GitHub Issues as the structured agent task queue for Hermes work.

Proposed routing

Librarian researches → Hero8 architects/plans → Hero7 or Copilot implements → Hero8 reviews → GuardianCOO audits when risk/safety/security requires it.

Acceptance criteria

• Agent Task issue template is available.
• Maintainers can create the recommended lane/status/agent/risk labels.
• Documentation explains issue quality bar, work-log standard, and approval gates.
• Agent-ready issues include objective, context/evidence, acceptance criteria, likely files, out-of-scope, risk, approval gates, and handoff instructions.
