Preflight Checklist

  • I have searched existing issues and this hasn't been reported yet
  • This is a single bug report (please file separate reports for different bugs)
  • I am using the latest version of Claude Code

What's Wrong?

Error message: Streamable HTTP error: Error POSTing to endpoint: MCP tool call requires approval

Environment: Claude Code on the web (cloud/remote execution)

What was tried in [.claude/settings.json] permissions.allow:

Wildcard patterns: mcp__list_events, mcp__read_file_content, etc. Exact human-readable names: mcp__Google-Calendar__list_events, mcp__Todoist__find-tasks-by-date, etc. UUID-prefixed names (copied from runtime tool names): mcp__c8585ad8-2e4b-4d71-9b14-cd8d45bdb660__list_events, etc. All three formats produce the same error. Tools load successfully (schemas resolve via ToolSearch) but every call is blocked regardless of allowlist configuration.

Affected MCP servers: Google Calendar, Todoist, Google Drive, Box

What Should Happen?

Expected behavior: Tools listed in permissions.allow should execute without prompting.

Another batch ranked right after the header list — different links, same matching logic.
