The Codex IDE extension appears to have introduced a paste behavior regression in version 26.527.31454.

When I paste a structured task card into the Codex input composer, the extension converts the pasted text into a file attachment named Pasted text.txt instead of inserting the pasted content as visible, editable text in the input box.

This started shortly after the extension auto-updated. I noticed the issue approximately 46 minutes after the update.

This is not only happening with extremely large prompts. In testing, partial task cards over roughly ~2,000 characters, possibly somewhat higher or lower, were converted into a file attachment. That is less than one-third of my normal task-card size.

There is no visible option that I could find to convert the attached Pasted text.txt back into editable composer text, such as Show in text field, Insert as text, or similar.

This breaks structured Codex task-card workflows because:

• the task card cannot be edited before submission
• I cannot easily scroll back through the visible task card during execution to review details or debug why a step is failing
• submitting only the attached pasted text causes Codex to ask what I want it to do with the file
• I then have to type a generic instruction such as Execute task card
• the Codex task/session name becomes generic, for example Execute task card, instead of using the actual branch/slice/task name
• multiple tasks become hard to distinguish in the task list
• action appears blocked while the pasted text file finishes uploading

Rolling back both the local and remote Codex IDE extension versions from 26.527.31454 to 26.519.32039 fixed the issue.

This appears to be a regression introduced between:

• Working version: 26.519.32039
• Broken version: 26.527.31454

1. Use VS Code on Windows.

2. Connect to a Linux workstation using VS Code Remote SSH over Tailscale.

3. Use the Codex IDE extension in the remote SSH workspace.

4. Install/use Codex IDE extension version 26.527.31454.

5. Paste a structured task card or long prompt into the Codex input composer.

   • In my testing, this occurred with pasted text over roughly ~2,000 characters.
   • The exact threshold may be higher or lower, but it is well below my normal task-card length.

6. Observe that the pasted text is converted into an attachment named Pasted text.txt instead of appearing as editable text in the composer.

7. Try to submit with only the attached pasted text.

8. Observe that Codex does not treat it as an executable task card and asks what I would like it to do with the file.

9. Type a separate generic instruction such as Execute task card.

10. Observe that the task/session name becomes the generic instruction instead of the actual task or branch/slice name.

Rollback test:

1. Roll back both the local and remote Codex IDE extension versions to 26.519.32039.
2. Repeat the same paste workflow.
3. The prior expected editable paste behavior is restored.

Pasted structured text should remain visible and editable in the Codex IDE composer, or the IDE should provide a clear escape hatch when it converts pasted text into an attachment.

Acceptable expected behavior would include one or more of the following:

• keep pasted text visible and editable in the input composer
• provide a visible Show in text field option
• provide an Insert as text or Convert attachment to prompt text option
• provide a setting to disable automatic pasted-text-to-file conversion
• preserve task/session naming from a clear task/slice/branch heading inside the pasted content
• make it clear before submission that Codex will treat the paste as a file attachment rather than as prompt instructions

Auto-filing very large pasted content can be reasonable, but without an obvious way to convert it back into editable prompt text, it becomes a workflow-breaking regression for structured development workflows.

The Codex CLI is installed only on the remote Linux machine and appears separate from this issue.

Local Windows did not have the Codex CLI installed.

Rollback result:

Rolling back both the local Windows-side and remote SSH-side Codex IDE extension installs to 26.519.32039 restored the expected editable-paste behavior. Disabling auto-update prevented the issue from returning.
