What's Wrong?

Claude Code frequently returns "API Error: Claude Code is unable to respond to this request, which appears to violate our Usage Policy" on completely normal coding requests.

This happens across multiple repositories and different types of prompts — none of which involve offensive security, harmful content, or policy violations. It appears to be a false positive from the content classifier.

What Should Happen?

Normal coding requests should receive normal responses without being blocked by the Usage Policy filter.

Steps to Reproduce

  1. Open Claude Code in any repository
  2. Make normal coding requests (e.g., "add an endpoint", "fix this function", etc.)
  3. Randomly, some requests are blocked with the Usage Policy error
  4. Retrying the same message or rephrasing sometimes works, confirming it's a false positive
