What's Wrong?

Two consecutive /ultrareview runs against the same branch crashed with:

Both runs were deducted from my free monthly quota (2 of 3 used) despite producing no findings.

Details:

Actual: Credit consumed, no findings returned. Crash happened both times on the same diff, suggesting a deterministic issue with the review input rather than a transient failure.

What Should Happen?

Review completes and returns findings, or credit is not consumed on crash.
