What's Wrong?

When you rename a conversation using /rename, the conversation tab name updates correctly, but the same conversation still shows the old name in the Sessions sidebar.

What Should Happen?

The Sessions tab should reflect the new conversation name immediately after renaming.

Steps to Reproduce

  1. Open Claude Code in VS Code
  2. Start or open a conversation
  3. Run /rename and give it a new name (e.g., "My Task")
  4. Check the Sessions tab on the right sidebar
  5. Observe: Tab shows "My Task", but Sessions tab shows the old/generic name
