When renaming a chat session in the VS Code sidebar (via the pencil icon), the new name reverts visually as soon as another session is clicked. The rename is saved — it appears correctly after closing and reopening VS Code — but the sidebar doesn't refresh in place.

Steps to reproduce

  1. Open the Claude Code sidebar in VS Code
  2. Click the pencil icon on any chat session
  3. Type a new name and press Enter
  4. Click a different chat session
  5. The renamed session reverts to its original name in the sidebar

Expected

Sidebar updates immediately after confirming the rename.

Actual

Name reverts until VS Code is fully closed and reopened.

Workaround

Close and reopen VS Code.
