No matter how many times I ONLY select "Yes, and don't ask again this session" for file edits, Codex still asks me again for every single file edit (in the same session)

This happens in all modes: Default permissions, Auto-review, Custom (config.toml)

It doesn't matter which I select, I still get asked to approve every single file edit
