Almost every conversation eventually shows the generic error screen:

"Oops, an error has occurred" This is not just a model error — it appears at the app level and blocks the whole conversation. Actual: The chat shows the "Oops, an error has occurred" screen. "Try again" rarely recovers it. Once a thread is in this state it often stays broken. Sometimes the error appears even on brand-new chats.

  • The error occurs even on simple, text-only prompts — not only when using the Chrome control feature.
  • Sign-out/sign-in provides temporary relief but the error returns as soon as you navigate away and back to a chat.

Environment

  • App: Codex Desktop
  • Version: 26.513.31313
  • OS: macOS 26.5 (25F71) "Tahoe"
  • Hardware: Mac (Apple Silicon)
  • Chrome extension: Codex Chrome extension installed and enabled

Issue

"Oops, an error has occurred" [Buttons: "Check for updates" and "Try again"]

This is not just a model error — it appears at the app level and blocks the whole conversation.

Steps to Reproduce

  1. Open Codex Desktop on macOS.
  2. Sign in normally.
  3. Start a new chat.
  4. Send a message (this usually works and gets a reply).
  5. Navigate away (e.g., click another conversation or change view).
  6. Navigate back to the previous chat.

Expected: The previous chat opens and I can continue the conversation.

Actual: The chat shows the "Oops, an error has occurred" screen. "Try again" rarely recovers it. Once a thread is in this state it often stays broken. Sometimes the error appears even on brand-new chats.

What I Already Tried

  • Signing out of Codex and signing back in (temporarily fixes it for one session).
  • Restarting the app and the Mac.
  • Checking for updates from inside the app (no newer version offered).
  • The error occurs even on simple, text-only prompts — not only when using the Chrome control feature.

Notes

  • Issue began after updating to 26.513.31313.
  • Sign-out/sign-in provides temporary relief but the error returns as soon as you navigate away and back to a chat.
  • Happy to provide logs or run a diagnostic build if needed.
