Codex Desktop completed a project task, but the conversation never received an auto-generated title. Even after the task finished, the conversation was still shown in the Desktop sidebar as New chat.

This is not just a title display problem on Codex mobile. The affected Desktop conversation does not appear in Codex mobile at all. Mobile is connected to the same Mac host and the same StepLens project, and other project conversations are visible there, but this specific finished Desktop New chat conversation is missing from the mobile conversation list.

Error Message

  1. In Codex Desktop, I ran a task under the StepLens project to compile/install the companion iOS app to my iPhone 16 Pro.
  2. The Desktop task completed and produced a final response.
  3. Even after completion, the Desktop sidebar still showed this conversation as New chat; no generated title appeared.
  4. In Codex mobile, I connected to the same Mac host and opened the same StepLens project.
  5. Codex mobile listed other conversations in that project, such as 连接手机 App 使用, 编译 iOS App 到真机, 搭建 iOS Companion 骨架, and others.
  6. The finished Desktop conversation that is still titled New chat was not shown anywhere in the mobile conversation list.

Root Cause

Codex Desktop completed a project task, but the conversation never received an auto-generated title. Even after the task finished, the conversation was still shown in the Desktop sidebar as New chat.

After a Desktop task finishes, the conversation should receive a generated title. Even if title generation fails or is delayed, Codex mobile should still list the conversation using a fallback title such as New chat, Untitled chat, or a timestamp-based placeholder.

Title generation state should not determine whether a conversation appears in the mobile conversation list.

Actual behavior

A completed Desktop conversation remained titled New chat, and that same conversation was completely absent from Codex mobile under the same host and same project.

Impact

This creates a Desktop/mobile history consistency issue. A finished development conversation can be visible on Desktop but unreachable from mobile, even though mobile is connected to the correct host and project.
