What issue are you seeing?

After a plan has already been accepted and Codex has started executing it, the iOS mobile UI can continue showing the “Implement this plan?” prompt.

This is not just a stale visual state. In the captured session, Codex is already running tool/read/search steps and the task checklist shows progress at 4/5 completed, but the mobile UI still displays the plan confirmation card. The prompt cannot be reliably closed/dismissed from the phone, so the user cannot continue steering the running task from iOS.

This is especially severe in goal mode, where the user depends on the mobile UI to supervise a longer-running task and give follow-up guidance. Once the stale plan prompt appears, the session becomes hard to control from mobile even though the agent is already executing.

What steps can reproduce the bug?

  1. Open Codex from the ChatGPT iOS app.
  2. Start or continue a Codex task that produces a plan.
  3. Accept the plan with “Yes, implement this plan”.
  4. Let Codex continue executing the task.
  5. After execution has started, observe the iOS UI showing the “Implement this plan?” prompt again or leaving it visible.
  6. Try to dismiss the prompt and continue guiding the running task from the phone.

Observed trigger: this only appears after a plan has been created/accepted. It is most disruptive in goal mode.

What is the expected behavior?

Once the user accepts a plan and Codex starts executing it:

  • The “Implement this plan?” prompt should be consumed and should not keep reappearing.
  • If the prompt is stale, closing/dismissing it from iOS should work.
  • The mobile UI should reflect the real execution state and allow the user to keep supervising or guiding the task.
  • Goal mode should not get blocked behind a stale plan confirmation prompt after execution has already started.
