Description
When using voice capture multiple times for prompting, an older transcribed voice recording is appended to the end of a new prompt. See the reproduction steps / video for more information.
What steps can reproduce the bug?

Configuration of codex with a computer (macbook in my case) and iOS ChatGPT app
Steps
- Open iOS ChatGPT app and go to codex
- Pick any chat or project chat and use the microphone to capture a voice prompt
- Using the tap shortcut or keyboard manually, delete the prompt
- Repeat step 2
You should see your older voice prompt appended to the end of your newer one.
What is the expected behavior?
When the user clears the transcribed text in the prompt and records a new voice prompt, the deleted one should not be added at the end.
