Bug Description Claude Code spent an entire day failing to complete a basic request, even when given explicit step-by-step instructions multiple times. Instead of making steady progress, it repeatedly lost context, erased work it had already completed, fixed one issue while undoing the original requested changes, and wasted hours cycling through low-value edits. In one case, it spent around four hours reverting or overwriting the very thing it had been asked to fix.

This raises a serious concern about the purpose and reliability of the max plan. A higher-tier plan should translate into better execution, stronger context retention, and fewer destructive regressions, not prolonged poor performance that consumes time and tokens without delivering a usable result.

The main issues were:

failure to retain prior instructions and completed work repeatedly undoing correct changes fixing one thing while breaking another excessive token consumption with little or no meaningful progress inability to either continue effectively or stop when stuck

Please review this behavior, fix the underlying execution and context-handling issues, and refund the tokens wasted on this failed effort.
