Bug Description

When renaming a session in the VS Code sidebar, the new title reverts to the original after clicking elsewhere, even after pressing Enter to confirm.

Steps to Reproduce

  1. Open Claude Code in VS Code sidebar
  2. Right-click a session → Rename
  3. Type a new session title
  4. Press Enter (title appears to save successfully)
  5. Click anywhere else in VS Code

Expected Behavior

The new session title should persist after pressing Enter.

Actual Behavior

The title reverts back to the previous/auto-generated title once focus leaves the rename field.

Environment

  • VS Code Extension: anthropic.claude-code v2.1.138
  • OS: macOS (Darwin 25.4.0)
  • VS Code: latest stable
