The current behavior breaks momentum and makes Codex feel unreliable. This needs a real engineering fix, not a workaround.”

What steps can reproduce the bug?

Example:

• User approves Bash/RTK shell usage.
• Subsequent similar commands still trigger more approval prompts.
• This makes normal repo work very disruptive.

What is the expected behavior?

Expected: Once a user grants approval for a reasonable command prefix/session scope, equivalent commands should run without repeated prompts.

Impact: Breaks coding flow and makes Codex difficult to use for local development.
